Requesting Medical Records

Sometimes, you need to obtain the patient's medical records from another health facility. Some are visible under "Care Everywhere" in Epic, but that has its limitations.

Usually, this is so we can have more information about an AE.

We do this by faxing a request form to the outside health facility.

In this folder,
U:\rex\shared\Clinical Research\NCHV Research\FORMS AND TEMPLATES

There lives a document called,
"MEDICAL RECORDS REQUEST FAX.doc"

That document is a template of a records request form. Amend it to fit your needs.

Scan that to the outside facility with a copy of the patient's ICF. This shows we have authorization to ask for the patient's records.

The turnaround time may vary. Follow up with outside facility via phone/email if needed.
